Diaphragmatic Rupture Secondary to Blunt Thoracic Trauma
نویسندگان
چکیده
Introduction: Diaphragmatic rupture is a relatively rare injury with a reported incidence of 1 to 7% of all patients following significant blunt trauma. 4 In a review of 1589 patients, Asensio et al reported that 75% had left sided injuries, 23% had right-sided injuries, and 2% had bilateral injuries. 1 This preponderance for left-sided injuries is thought to be related to the protective effect of the bare area of the liver in contact with the diaphragm in the right thorax. 3 The pathophysiology in blunt trauma is due to the abrupt change in intraabdomonial pressure that is thought to cause the majority of injuries, although shearing and/or avulsion can also occur especially following lateral trauma. 2

Background: The diaphragm is the main respiratory muscle; the diaphragm can rupture in the field of blunt trauma, penetrating trauma or occur following medical procedures. Approximately 0.8 to 1.6% of patients with blunt trauma are suspected to have a rupture of the diaphragm. As diaphragmatic rupture is a rare clinical condition, so it may be undiagnosed and associated with complications and h...

Diaphragmatic injury is often a missed diagnosis in patients with multiple trauma. For this reason, mortality can be high. From 1970 to 1981, 32 patients with diaphragmatic injuries were seen at Maisonneuve-Rosemont Hospital. Twenty-four of the patients (22 men and 2 women aged 18 to 79 years) had blunt abdominal or thoracic trauma causing diaphragmatic disruption. Rupture occurred 20 times on ...
متن کاملAcquired Diaphragmatic Hernia: A Case Report
Diaphragmatic hernias can be described as congenital hernias or traumatic hernias (including those following penetrating trauma, blunt trauma and unrecognized diaphragmatic rupture) [1]. A blunt trauma to chest or abdomen is the main cause of diaphragmatic injury. This injury occurs in 1.1-3.9% of the cases following a trauma [2,3]. Then, acquired diaphragmatic hernia (ADH) occurs in less than ...
متن کاملLaparoscopic Treatment of a Rare Right Diaphragmatic Rupture with Small Bowel Herniation after Blunt Thoracic Trauma
Blunt traumatic diaphragmatic rupture (BTDR) is a life-threatening condition with an incidence from 0,8%-1,6% in blunt trauma, mostly located on the left side. The main prognostic factors are severe side injuries and the delay of diagnosis. We present a rare case of a 68-year-old female, with an isolated right diaphragm rupture. The diagnosis was done with a delay of 4 days by thoracic radiogra...
متن کاملDiaphragmatic rupture with right colon and small intestine herniation after blunt trauma: a case report
INTRODUCTION Traumatic diaphragmatic hernias are an unusual presentation of trauma, and are observed in about 10% of diaphragmatic injuries. The diagnosis is often missed because of non-specific clinical signs, and the absence of additional intra-abdominal and thoracic injuries. CASE PRESENTATION We report a case of a 59-year-old Italian man hospitalized for abdominal pain and vomiting. His m...
